Partner Insurance & Loss Runs Checklist

For Tier 1: $1,000,000 ($1M) limit for General Liability, Automobile, Umbrella, Errors and Omissions/Professional Liability, and Workers Comp, there are (10) overarching requirements on our checklist.

Refer to your LightReach Channel Partner Agreement, Exhibit 4, for policy line details.

Company Name

As it appears on any insurance related documents must EXACTLY match their legal name as it appears on the W-9

Carrier Rating

Must be rated a ‘A’ or better by A.M. Best Company, whose ratings may be viewed at http://www.ambest.com.

Coverage Limits

  1. General Liability: must be occurrence based. General Aggregate, Products Comp/Op Aggregate and Each Occurrence limits must meet or exceed the level specified by Palmetto

  2. Auto Liability: must be specified (any auto, all owned autos, scheduled autos, hired autos or non-owned autos). Limits must be in force either for a Combined Single Limit or Bodily Injury (per person). Bodily Injury (per accident) and Property Damage. Limits must meet or exceed the level specified by Palmetto

  3. Umbrella

  4. Workers’ Compensation: must comply with the statutory limits set by your state law. Partner’s liability coverage must be in force for Each Accident, Disease-Policy Limit and Disease-Each Employee. Limits must meet or exceed the level specified by Palmetto. Partner must indicate whether any of its’ principals are included under or excluded from workers’ compensation coverage.

  5. Errors and Omissions/Professional Liability: $1,000,000 per occurrence with a $3 million aggregate

Availability

Current insurance certificates must be provided to Palmetto at all times

Policy Number

All insurance certificates must list the policy number of each line of coverage.

Certificate Holder

All insurance certificates must list the following as the certificate holder:

Signatures

Insurance agent must sign all insurance certificates

Cancel Statement

All insurance certificates should include the cancellation statement: Should any of the above-described policies be cancelled before the expiration date thereof, notice will be delivered in accordance with the policy provisions.

General

Liability

The occurrence box must be checked for General Liability

Required Statement

All insurance certificates (except any Workers’ Compensation Certificate of Insurance) must contain the following statement:

  1. Project and owner(s) are additional insured as respects to general liability, auto liability and excess liability as required by written contract. General Liability coverage for additional insured applies on a primary and non-contributory basis and includes completed operations. Waiver of subrogation applies in the favor of Additional Insured with respect to General Liability, Automobile Liability, Excess Liability and Workers; Compensation policies as required by written contract and as permitted by law. Workers’ Compensation complies with the state statures in which the work will be performed.
